ABSTRACT

Objective To analyze the research hotspots and theme in the field of infection prevention and control(infection control) management in operating room in recent six years in China, so as to provide reference for further research on infection control management in operating room. Methods The articles related to infection control management in operating room published in China National Knowledge Infrastructure(CNKI), Wanfang and VIP databases from 2015 to 2021 were selected as the research objects, and Nvivo software query, coding and cluster analysis were adopted as the main methods to visually present the research hotspots and theme groups in the field of infection management in operating rooms. Results A total of 128 literatures with 30 high-frequency keywords were included. After free coding, 8 parent nodes, 28 child nodes, 60 secondary nodes and 1 736 coding reference points were formed. Six theme groups had been formed, which were the guidelines for infection control of COVID-19 infection, construction and management of clean operating rooms, management of relevant personnel in operating rooms, management of surgical supplies, operating room environment control, theoretical support and occupational exposure control. Conclusion The research content related to infection control management in operating rooms is becoming increasingly mature, and is constantly improving in line with the development of the times. The research direction and theory are also showing a trend of diversification. In response to the huge challenge of COVID-19 situation, it is supposed to preparing for surgery protection in general hospitals actively, ensure the safety of patients and medical staff, and prevent hospital infection events. ABSTRACT

Objectives: Opioids play a significant role in the effective management of cancer-related pain. The COVID-19 lock down may have reduced access to opioids and caused a decline in the use of prescription of opioids among cancer survivors. This study compared opioid prescription rates among cancer survivors before and after the onset of COVID-19 pandemic using real-world electronic health records (EHR). Method(s): Cohort analyses of cancer patients using data from EHR database from the TriNetX, a global federated health research network across 76 healthcare organizations. We analyzed changes in prescription opioid use before (March 1, 2018, through March 1, 2019) and after onset of COVID-19 (April 01, 2020, through March 2021) among cancer survivors. The key outcome variable was any opioid prescription within 1 year of cancer diagnosis. One-to-one propensity score matching was used to balance the characteristics (age, sex, race, diagnoses including diabetes, hypertensive diseases, overweight, mood disorders, and visual disturbances) of the two cohorts. Data were analyzed using the TriNetX platform. Result(s): There were 1,502,143 cancer survivors before COVID-19 and 1,412,599 cancer survivors after the onset of COVID-19. The one-to-one propensity-score match yielded 1,382,561 cancer patients, mean age 64 at cancer diagnosis, and 73% were white. Percentage of opioid use among cancer patients declined from 35.6% before the COVID-19 to 35.1% after the onset of the pandemic (OR=0.976, 95% CI 0.971-0.981). Average number of opioid prescriptions within 1 year of cancer diagnosis declined from 5.7 before to 5.3 after the COVID-19 onset (p<0.001). Conclusion(s): Among cancer survivors, a small decline in prescription opioid use was observed after the onset of COVID-19 pandemic. Future studies are needed to distinguish the impact of revised guidelines, opioid prescription policy changes, and COVID-19 lock down on lower rates of prescription opioid use among cancer survivors.Copyright © 2023

ABSTRACT

Objectives: To evaluate how payers utilize Institute for Clinical and Economic Review (ICER) assessments to inform coverage or formulary decisions. Method(s): Double-blinded, web-based survey was fielded through Xcenda's research panel, the Managed Care Network, from June to July 2022. Result(s): A total of 51 payers from health plans (n=27), integrated delivery networks (n=12), and pharmacy benefit managers (n=12) participated in the survey. When assessing the usefulness of ICER's value assessment framework (VAF) to inform formulary decisions within their organizations, 57% of payers indicated it was extremely/very useful, 33% indicated somewhat useful, and 10% indicated not at all/not very useful. Most respondents (73%) agreed that ICER assessments are aligned with their organization's internal assessment. Utilization of ICER's VAF was most prevalent in high-cost drug or disease states (78%), rare/orphan disease states (71%), and oncology/hematology disease states (67%). Payers reported less use in primary care disease states (29%), COVID-19 (8%), and digital therapeutics (4%). In the last 24 months, 20% of payers reported ICER's recommendations often influenced coverage decisions, 59% indicated occasional influence, and 22% indicated no influence. In the last 24 months, payers indicated the top 5 ICER assessments that influenced their coverage decisions included high cholesterol (38%), Alzheimer's disease (36%), atopic dermatitis (33%), multiple myeloma (31%), and chemotherapy-induced neutropenia (28%). ICER assessments that were less impactful included beta thalassemia (3%), digital health technologies (3%), and supervised injection facilities (3%). Payers reported using ICER assessments to inform both expanded and restricted coverage decisions. Conclusion(s): Payers find ICER's VAF useful to inform their organization's formulary decisions. ICER's assessments often align with payers' internal assessments and are most frequently utilized for high-cost drugs or disease states. Payers indicate ICER assessments have affected both expansion and restriction in their coverage policies.Copyright © 2023

ABSTRACT

The World Health Organization has labeled the novel coronavirus illness (COVID-19) a pandemic since March 2020. It's a new viral infection with a respiratory tropism that could lead to atypical pneumonia. Thus, according to experts, early detection of the positive cases with people infected by the COVID-19 virus is highly needed. In this manner, patients will be segregated from other individuals, and the infection will not spread. As a result, developing early detection and diagnosis procedures to enable a speedy treatment process and stop the transmission of the virus has become a focus of research. Alternative early-screening approaches have become necessary due to the time-consuming nature of the current testing methodology such as Reverse transcription polymerase chain reaction (RT-PCR) test. The methods for detecting COVID-19 using deep learning (DL) algorithms using sound modality, which have become an active research area in recent years, have been thoroughly reviewed in this work. Although the majority of the newly proposed methods are based on medical images (i.e. X-ray and CT scans), we show in this comprehensive survey that the sound modality can be a good alternative to these methods, providing faster and easiest way to create a database with a high performance. We also present the most popular sound databases proposed for COVID-19 detection. © 2022 IEEE.

ABSTRACT

The multi-generational workforce presents challenges for organizations, as the needs and expectations of employees vary greatly between different age groups. To address this, organizations need to adapt their development and learning principles to better suit the changing workforce. The DDMT Teaching Model of Tsing Hua STEAM School, which integrates design thinking methodology, aims to address this challenge. DDMT stands for Discover, Define, Model & Modeling, and Transfer. The main aim of this study is to identify the organization development practices (OD) and gaps through interdisciplinary models such as DDMT and design thinking. In collaboration with a healthcare nursing home service provider, a proof of concept using the DDMT-DT model was conducted to understand the challenges in employment and retention of support employees between nursing homes under the healthcare organization. The paper highlights the rapid change in human experiences and mindsets in the work culture and the need for a design curriculum that is more relevant to the current and future workforce. The DDMT-DT approach can help organizations address these challenges by providing a framework for HR personnel to design training curricula that are more effective in addressing the issues of hiring and employee retention. By applying the DDMT-DT model, HR personnel can better understand the needs and motivations of the workforce and design training programs that are more relevant to their needs. The proof-of-concept research pilot project conducted with the healthcare nursing home service provider demonstrated the effectiveness of the DDMT-DT model in addressing the issues of hiring and employee retention. The project provides a valuable case study for other organizations looking to implement the DDMT-DT model in their HR practices. Overall, the paper highlights the importance of adapting HR practices to better suit the changing workforce. The DDMT-DT model provides a useful framework for organizations looking to improve their HR practices and better address the needs of their workforce.

With frequent outbreaks of COVID-19, the rapid and effective construction of large-space buildings into Fangcang shelter hospitals has gradually become one of the effective means to control the epidemic. Reasonable design of the ventilation system of the Fangcang shelter hospital can optimize the indoor airflow organization, so that the internal environment can meet the comfort of patients and at the same time can effectively discharge pollutants, which is particularly important for the establishment of the Fangcang shelter hospital. In this paper, through the reconstruction of a large-space gymnasium, CFD software is used to simulate the living environment and pollutant emission efficiency of the reconstructed Fangcang shelter hospital in summer under different air supply temperatures, air supply heights and exhaust air volume parameters. The results show that when the air supply parameters are set to an air supply height of 4.5 m, an air supply temperature of 18 °C, and an exhaust air volume of a single bed of 150 m3/h, the thermal comfort can reach level I, and the ventilation efficiency for pollutants can reach 69.6%. In addition, the ventilation efficiency is 70.1% and 70.3% when the exhaust air volume of a single bed is continuously increased to 200 and 250 m3/h, which can no longer effectively improve the pollutant emission and will cause an uncomfortable blowing feeling to patients. © 2023 by the authors.

ABSTRACT

Social media platforms such as Twitter provide opportunities for governments to connect to foreign publics and influence global public opinion. In the current study, we used social and semantic network analysis to investigate China's digital public diplomacy campaign during COVID-19. Our results show that Chinese state-affiliated media and diplomatic accounts created hashtag frames and targeted stakeholders to challenge the United States or to cooperate with other countries and international organizations, especially the World Health Organization. Telling China's stories was the central theme of the digital campaign. From the perspective of social media platform affordance, we addressed the lack of attention paid to hashtag framing and stakeholder targeting in the public diplomacy literature. ABSTRACT

By early 2020, COVID-19 has caused a global pandemic which led to an enormous number of challenges worldwide in various sectors. The Philippine government has implemented multiple quarantine guidelines and travel restrictions to ensure the people's health and safety. However, the International Labour Organization projected an initial economic and labor market disruption affecting 11 million workers, or about 25% of the Philippine workforce, due to the pandemic. Therefore, the government, thru the concerned agencies continues to encourage employers to implement alternative work plans such as a work-from-home (WFH) operation in compliance with the established regulations in line with existing laws and policies. In line with the telecommuting concept, various research has already been performed, however, some were regarded inconclusive and require further study. Hence, in this study, a Web application was developed along with an embedded fuzzy model to evaluate the telecommuting capability assessment of employees. The proposed web application with embedded fuzzy model is capable of providing capability assessment using the four main input variables which are also relatively characterized for possible telecommuting cost assessment. © 2022 IEEE.

ABSTRACT

Violations in the provision of medical care are detected by controlling the volume, timing, quality and conditions of medical care. The objective of the work is assessment of the activities of the round-the-clock hospital from the standpoint of the quality of medical care during the COVID-19 pandemic. Material and methods. A database of indicators reflecting the activity of round-the-clock hospital for four years, from 2017 to 2020 (Omsk) was formed. Analytical, statistical methods and the method of expert assessments were used to analyze the required indicators. Results. Evaluation of inpatient hospital activities during the COVID-19 pandemic compared to the "pre-pandemic” years showed a 1.9-fold and 2.9-fold decrease in the total number of quality examinations performed and the number of defects detected, respectively. The number of defects that were grounds for denial or reduction of payment for medical care found in the "before-COVID-19” years was 2.1 times higher than in 2020. In 2020, the weight of the share of those defects that directly reflect the quality of medical care increased significantly. The quantitative composition of the inpatient hospital staff changed insignificantly during the analyzed period – the number of physicians increased by only 5% over four years, with no dynamics in the qualitative characteristics of the staff. The number of medical equipment increased by 27.6%. Due to re-profiling in 2020, there was a decrease in the number of hospitalized patients, with an increase in the proportion of patients admitted by emergency care (up to 91%) and an increase in mortality by 2.7 times. Conclusions. One of the conditions for maintaining high quality of medical care is a balance between the main components of quality: accessibility, timeliness, sufficiency, efficiency and safety and the speed of its achievement, which in turn depends on the amount of resources and reserves. © 2023, Institute of Cytology and Genetics of Siberian Branch of the Russian Academy of Sciences. The pandemic of the COVID-19 has generated changes in the form of work in the organizations, with the Use of technologies of the information and the communication have been generated models like the telework that contribute diverse benefits to the companies. This review article aims to describe the advantages of telework in the hybrid model in organizations. The benefits of the hybrid model were obtained as the perfect formula to balance the results and minimize the disadvantages of pure telework. It is concluded that the organizations must generate the metrics of the workers in order to be able to measure their productivity no matter in which physical space they are.

ABSTRACT

Objectives: The Covid-19 pandemic highlighted the importance of considering Social Determinants of Health (SDoH) in healthcare research. Administrative claims databases are widely used for research, but often lack SDoH data or sufficient transparency in how these data were obtained. This study describes innovative methods for integrating SDoH data with administrative claims to facilitate health equity research. Method(s): The HealthCore Integrated Research Database (HIRD) contains medical and pharmacy claims from a large, national US payer starting in 2006 and includes commercial (Comm), Medicare Advantage (MCare), and Medicaid (MCaid) populations. The HIRD includes individually identifiable information, which was used for linking with SDoH data from the following sources: national neighborhood-level data from the American Community Survey, the Food Access Research Atlas, and the National Center for Health Statistics' urbanicity classification;and member-level data on race/ethnicity from enrollment files, medical records, self-attestation, and imputation algorithms. We examined SDoH metrics for members enrolled as of 05-July-2022 and compared them to the respective US national data using descriptive statistics. We also examined telehealth utilization in 2022. Result(s): SDoH data were available for ~95% of currently active members in the HIRD (Comm/MCare/MCaid 12.5m/1m/7.6m). Socioeconomic characteristics at the neighborhood-level differed by membership type and vs. national data: % of members with at least a high-school education (90/88/84 vs. 87);median family income ($98k/$76k/$70k vs. $82k);% of members living in low-income low-food-access tracts (9/14/18 vs. 13);urban (57/52/47 vs. 61). At the member-level, the % of White Non-Hispanics, Black Non-Hispanics, Asian Non-Hispanics, and Hispanics were 61/6/5/6 (Comm), 76/12/2/2 (MCare), and 45/26/5/19 (MCaid). Imputation contributed 15-60% of race/ethnicity values across membership types. Telehealth utilization increased with socioeconomic status. Conclusion(s): We successfully integrated SDoH data from a variety of sources with administrative claims. SDoH characteristics differed by type of insurance coverage and were associated with differences in telehealth utilization.Copyright © 2023

ABSTRACT

Sustainable agriculture is the backbone of food security systems and a driver of human well-being in global economic development (Sustainable Development Goal SDG 3). With the increase in world population and the effects of climate change due to the industrialization of economies, food security systems are under pressure to sustain communities. This situation calls for the implementation of innovative solutions to increase and sustain efficacy from farm to table. Agricultural social networks (ASNs) are central in agriculture value chain (AVC) management and sustainability and consist of a complex network inclusive of interdependent actors such as farmers, distributors, processors, and retailers. Hence, social network structures (SNSs) and practices are a means to contextualize user scenarios in agricultural value chain digitalization and digital solutions development. Therefore, this research aimed to unearth the roles of agricultural social networks in AVC digitalization, enabling an inclusive digital economy. We conducted automated literature content analysis followed by the application of case studies to develop a conceptual framework for the digitalization of the AVC toward an inclusive digital economy. Furthermore, we propose a transdisciplinary framework that guides the digitalization systematization of the AVC, while articulating resilience principles that aim to attain sustainability. The outcomes of this study offer software developers, agricultural stakeholders, and policymakers a platform to gain an understanding of technological infrastructure capabilities toward sustaining communities through digitalized AVCs.

ABSTRACT

Background/Objectives: Li-Fraumeni Syndrome (LFS) is a rare hereditary cancer predisposition syndrome characterized by high lifetime risks for multiple primary malignancies. Although most individuals with LFS inherit a pathogenic TP53 variant from a parent, approximately 20% have de novo variants with no suggestive family cancer history. This may result in an LFS experience distinct from individuals with affected relatives. This multi-case study report examines the unique psychosocial experiences of three young adults with de novo TP53 variants. Method(s): The National Cancer Institute's LFS study (NCT01443468) recruited adolescents and young adults (AYAs;aged 15-39 years) with LFS for qualitative interviews. Three participants had a de novo TP53 variant and a personal cancer history. An interprofessional team analyzed interview data using extended case study and narrative methods. Result(s): De novo participants lacked familiarity with LFS to situate a cancer diagnosis, interpret genetic test results, or adjust to chronic cancer risk. Communicating with and receiving support from family was challenged by their lack of common experience. De novo participants experienced socioemotional isolation, which was amplified during the COVID-19 pandemic. To cope, they sought support in online rare disease communities or through mental health providers. Conclusion(s): Individuals with de novo variants may lack familial guides and familiar providers to address disease management and uncertainty. Specialty health and mental health providers may support de novo patients across hereditary cancer syndromes by validating their uncertainties and connecting them with diseasespecific patient advocacy groups that support adjustment to chronic cancer risk.

ABSTRACT

Background: Nurses and other first responders are at high risk of exposure to the SARS-CoV2 virus, and many have developed severe COVID-19 infection. A better understanding of the factors that increase the risk of infection after exposure to the virus could help to address this. Although several risk factors such as obesity, diabetes, and hypertension have been associated with an increased risk of infection, many first responders develop severe COVID-19 without established risk factors. As inflammation and cytokine storm are the primary mechanisms in severe COVID-19, other factors that promote an inflammatory state could increase the risk of COVID-19 in exposed individuals. Alcohol misuse and shift work with subsequent misaligned circadian rhythms are known to promote a pro-inflammatory state and thus could increase susceptibility to COVID-19. To test this hypothesis, we conducted a prospective, cross-sectional observational survey-based study in nurses using the American Nursing Association network. Method(s): We used validated structured questionnaires to assess alcohol consumption (the Alcohol Use Disorders Identification Test) and circadian typology or chronotype (the Munich Chronotype Questionnaire Shift -MCTQ-Shift). Result(s): By latent class analysis (LCA), high-risk features of alcohol misuse were associated with a later chronotype, and binge drinking was greater in night shift workers. The night shift was associated with more than double the odds of COVID-19 infection of the standard shift (OR 2.67, 95% CI: 1.18 to 6.07). Binge drinkers had twice the odds of COVID-19 infection of those with low-risk features by LCA (OR: 2.08, 95% CI: 0.75 to 5.79). Conclusion(s): Working night shifts or binge drinking may be risk factors for COVID-19 infection among nurses. Understanding the mechanisms underlying these risk factors could help to mitigate the impact of COVID-19 on our at-risk healthcare workforce.Copyright © 2023 The Authors. Aims: The COVID-19 pandemic presented new difficulties for integrated healthcare worldwide. Our study aims to highlight developing needs for cooperation while describing structures and practices of consultation liaison (CL) services established during pandemic across Europe. Method(s): The cross-sectional survey used an ONLINE self-developed 25-item questionnaire in four language versions (English, French, Italian, German). Dissemination was via national professional societies cooperating in EAPM, and heads of CL services from June to October 2021 Results: 259 hospital CL services took part in the study (28.0% response rate). 222 (85.7%) of these services reported providing COVID-19-related mental health care (COVIDpsyCare). Among them, 192 services (86.5%) reported the development of specialized COVID-psyCare co-operation arrangements. 135 services (50.8%) provided specific COVID-psyCare for patients, 85 (38.2%) for relatives, and 171 (77.0%) for staff, with 56.3%, 14.6%, and 23.7% of time resources invested for these groups, respectively. Interventions for hospital staff, commonly related to the liaison function of CL services, were rated as being the most helpful. Regarding newly emerging demands, 129 (58.1%) CL services stated a need for communication and support among themselves concerning COVID-psyCare, and 142 (64.0%) suggested certain adjustments or enhancements that they thought were crucial for the future. Conclusion(s): Specific structure to provide COVID mental health care for patients, their relatives, or staff were implemented in over 80% of the participating CL services. Resources were primarily allocated for patient care, and staff assistance was mostly achieved through the implementation of specialized interventions. COVID psyCare's development calls for further intra- and interinstitutional cooperation.Copyright © 2023

ABSTRACT

The world is becoming a place where the number of emergencies and humanitarian crises is increasing rapidly due to economic inequality and the gap between developed and underdeveloped countries, as well as climate changes leading to disruption of the natural balance and development of natural disasters. The most vulnerable groups of the population including women and children always are affected by disasters. The younger the child, the more vulnerable he/she is, especially if not naturally fed or having a mother or parents. Various humanitarian organizations have been involved in a number of crises, with the World Health Organization and UNICEF and other United Nations-related organizations leading the way. In the care of mothers, infants and young children, most important is to ensure appropriate nutrition because otherwise it can result in life-threatening health conditions. The lack of protection, support and promotion of natural nutrition (breastfeeding) and its disruption and undermining by uncritical and uncontrolled donations and distribution of infant formula are the biggest challenge due to the lack of information of mothers, those who provide support in emergencies from both governmental and non-governmental sector, without cross-sectoral cooperation, thus causing uncoordinated and sometimes harmful interventions. Therefore, it is recommended that governments issue guidelines on infant and young child nutrition prior to the occurrence of an emergency, and crisis management regulations in which the issue of infant and young child nutrition will be given due consideration.Copyright © 2020 Croatian Paediatric Society.
